Home Health Aide

They coordinate with your loved one’s doctor to fully understand their needs so they can provide the best service.
PASSPORT Services
We help your loved ones secure all the requirements they will need to enroll in the PASSPORT program so they can access customized health care services in the comfort of their home. Determine if your loved ones can qualify for the program if they match the primary criterion issued by PASSPORT.
Qualifications
- You must be at least 60 years of age or older.
- Your medical assessment declares you require the same level of care as someone in a nursing home.

DODD
We work with the Department of Developmental Disability (DODD) to support clients and formulate person-centered healthcare services that will meet all their special needs. Our staff is all certified and accredited to ensure safe and high-quality service for your loved ones.
MRDD
Supporting families with loved ones who are suffering from Mental Retardation and/or Developmental Disabilities (MRDD) and working alongside them to develop a home health care plan that fully accommodates their loved one’s needs. Ensuring they can live their lives comfortably at home.
